Mobile Application for Chronic Disease Management: a Case of Diabetes

Abstract
Chronic diseases, particularly diabetes, require constant monitoring, often managed through inefficient paper-based records and periodic clinic visits. This research aims to develop a mobile application to improve diabetes management by addressing non- adherence to medication, complex schedules, limited health literacy, and insufficient continuous support. The app is designed using UML notations and implemented using PHP and MySQL database. By enhancing data accuracy, providing real-time monitoring, and empowering patients with knowledge and tools for effective self- management, the application aims to transform chronic disease management, reduce healthcare costs, and improve patient outcomes. This study underscores the potential of mobile health (mHealth) in modern healthcare.
