?ABSTRACT?Health record documentation is pivotal in ensuring quality patient care and effective healthcare audits. This study investigates the evaluation of health records documentation on health care audit within the Obafemi Awolowo University Teaching Hospitals Complex (OAUTHC), Ile-Ife, Nigeria. Using a cross-sectional design, data were collected from 120 healthcare professionals through structured questionnaires. The study aimed to assess the current state of documentation practices, evaluate the utilisation of health record data for audits, and examine the influence of documentation on patient care quality. Descriptive and inferential statistical analyses were conducted to identify relationships between documentation quality and audit effectiveness.?Findings reveal that while documentation practices at OAUTHC are generally consistent, there are notable gaps in completeness and accuracy, which impact audit processes and patient care outcomes. Effective utilisation of health records for audits was found to enhance the identification of clinical errors and improve patient management strategies. However, challenges such as time constraints, inadequate training, and inconsistent documentation standards were identified as significant barriers. The study concludes that improving documentation practices through targeted training, standardisation of processes, and the adoption of advanced electronic health record systems could significantly enhance healthcare audit effectiveness and patient care quality. Recommendations include implementing regular training programs for healthcare providers and integrating user-friendly electronic health record systems to facilitate better documentation practices.?
