Online Health Information Record Case of Study Aminu Kano Teaching Hospital

Abstract
This project focuses on the design and implementation of an Online Health Information Record System aimed at improving the efficiency, accuracy, and accessibility of patient data management in healthcare institutions, particularly at Aminu Kano Teaching Hospital. The study identifies the major challenges associated with traditional paper-based health record systems, such as data loss, redundancy, difficulty in retrieval, and security risks. The system developed in this project provides a centralized digital platform for recording, storing, and retrieving patient information in real time. It incorporates modules for patient registration, medical history tracking, diagnosis documentation, and report generation. The system was designed using PHP, MySQL, HTML, CSS, and JavaScript, with emphasis on data security through user authentication and role-based access control. The findings of the study show that the implementation of an online health information record system significantly enhances data management efficiency, reduces manual errors, and improves decision-making by providing timely and accurate information to healthcare providers.
